About Transition Care and Restorative Care Programs
The Transition Care and Restorative Care Programs sits within Aged Care Community Services as part of the Continuing Care Division at Austin Health. Transition Care provides short-term interventions for older people at risk of inappropriate extended hospital stays and/or at risk of premature admission to residential care. Restorative Care is an inpatient program located in our partner Aged Care Facility designed to support clients who are under 65, or who are non-weightbearing. Programs are goal based and can be provided in residential and community settings.
About the role
As the Team Leader you will provide operational oversight and support to the Transition Care and Restorative Care Programs and staff. Supported by the Community Aged Care Services Manager you will have a key role in supporting a positive team culture, overseeing the intake and bed flow functions and ensuring program achievement of KPIs. The role encompasses a clinical component to maintain expertise.
A regular day will see you:
- Overseeing a team of case managers and assessors, providing guidance, supporting assessment and discharge planing, and fostering a positive and supportive working environment
- Stakeholder engagement to support internal relationships, organisational bed flow and community connections
- Working closely with the Community Aged Care Services Manager to support program effectiveness and achievement of reportable KPI's
About you
You will be someone who has well developed leaderships skills. You’ll have a passion for Aged Care Services, a strong commitment to delivering high quality care. You’ll have a strong focus on quality and service improvement, and a proven ability to support teams through change. An understanding of the aged care reforms would be beneficial.
You’ll further bring with you:
- Current registration as a Registered Nurse (currently registered with Nursing and Midwifery Board of Australia) or Allied Health Professional (with current registration with their respective National Board) or eligible for membership of relevant professional association
- Excellent interpersonal and communication skills that support team engagement, patient care, and collaboration
- A willingness to think outside the square and think creatively to achieve the best possible program and client outcomes
- Demonstrated flexibility to change and adapt with program and organisational requirements
