Social Worker (Grade 2)

About the role:

If you have experience working with vulnerable people and individuals who require support for complex discharge planning, if you have skills assessing and responding to Family Violence and Child Safety risk, and / or knowledge about how to navigate NDIS and Aged Care systems, then we want you to join our team!

 

As a Grade 2 Social Worker you will be responsible for providing clinical social work services to inpatients across one or more of our Austin Health sites. A regular day will see you work clinically with patients, attend family meetings and non-clinical meetings, and undertake other duties that support the social work department and its operations.

 

Currently, we have positions in several streams with a focus on Oncology, Surgical and Permanent Leave cover across all sites, please indicate your preference in your cover letter. 

 

Key responsibilities will see you:

  • Apply your social work knowledge and skills to assess, treat and plan the discharge of patients
  • Provide counselling to patients and families
  • Supervise Grade 1 social workers and social work students 
  • Participate in quality improvement, service improvement and/or research

 

About you

You will be someone who has excellent communication skills and a strong commitment to excellence in patient care. You’ll bring energy and drive to you work, and have a keen interest in developing your skillset.

 

You’ll further have:

  • A degree in Social Work
  • Worked in a hospital or health related setting
  • The ability to work and contribute to team culture and work within a multidisciplinary environment
