About the department:
The Social Work department is committed to promoting the highest level of care for our patients. In realising this goal, the department leads and excels in person-centred care, evidence-based practice, innovation and quality improvement. The department provides Social Work services on all three campuses of Austin Health, from the Emergency Department through to bed-based services and ongoing ambulatory care.
About the role:
As a Grade 2 casual Social Worker you will be responsible for providing clinical Social Work services to inpatients across one or more of our Austin Health sites. There are weekend and weekday shifts available. A regular day will see you work clinically with patients, attend family meetings and non-clinical meetings, work within a multidisciplinary team, and undertake other duties that support the Social Work department and its operations.
Key responsibilities will see you:
- Apply your Social Work knowledge and skills to assess, treat and plan the discharge of patients
- Undertake comprehensive psychosocial assessments, and develop and implement appropriate intervention plans, including care planning, counselling, and discharge planning for patients and their families.
- Deliver effective Social Work interventions to patients and families in a range of settings and ensure assessment and intervention is aligned with professional values, skills, knowledge, theory, relevant policies and legislation.
About you:
You will be someone who has excellent communication skills and a strong commitment to excellence in patient care. You’ll bring energy and drive to you work and have a keen interest in developing your skillset.
You’ll further have:
- A degree in Social Work
- Experience working in a hospital setting.
- The ability to work and contribute to team culture and work within a multidisciplinary environment
