About the department
Our Community Rehab Services team is a large interdisciplinary team providing home, telehealth and centre based therapy to a broad range of patient populations. This dynamic position may be based across both Repatriation and Royal Talbot campuses.
About the role
As Grade 2 Social Worker in the Community Rehab Services team, you will be carrying a varied caseload mix of community clients such as neurological, orthopaedic, aged care, amputee or general rehabilitation clients.
A regular day will see you:
- Work with the interdisciplinary team to plan, discuss and treat clients
- Complete comprehensive psychosocial assessments of clients and their family/support systems
- Educate and support the healthcare team to the social and emotional aspects and impacts of a client’s condition
- Provide goal centred and client centred therapeutic interventions
- Be involved in team meetings, PD and quality projects
About you
You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work and have a keen interest in developing your skillset.
You’ll further bring with you:
- Educational Qualifications and Training
- Bachelor of Social Work degree or Master of Social Work (qualifying)
- A minimum of 3 years of experience in Social Work assessment and intervention, predominantly in the area of adult, aged care & rehabilitation, e.g. Neurological, Musculoskeletal.
