About the Pharmacy department
The Pharmacy Department Mission Statement is: "Working together to provide optimum pharmaceutical care". Austin Health Pharmacy Department provides comprehensive high-quality care to Austin Health patients and supports other members of the healthcare team in all aspects of medicines management. The Department consists of over 120 full time equivalent staff over 3 campuses within Austin Health. The team consists of pharmacists and technical and support personnel who work together to ensure patients receive the highest quality pharmaceutical care possible.
About the role
This position is responsible for the oversight and leadership for the provision of pharmacy services to Hospital in the Home (HITH), Virtual Care and Ambulatory Care Centre patients.
This role will see you:
- Oversee daily pharmacy services for HITH and Virtual Care, ensuring medication management, policy updates, and coordination of services in line with hospital and legal standards
- Provide clinical support, including medication monitoring, liaising with staff, maintaining drug regimens and pump libraries, and assisting in the development of a 7-day service for HITH and Virtual Care
- Manage day-to-day activities for the provision of pharmacy services to Ambulatory Care Centre and Residential Outreach Service
- Provide leadership, supervision and support to the HITH and Virtual Care team
- Facilitate training and education for interns, undergraduates, and staff, including orientation, competency assessments, professional development, and performance reviews.
About you
As the Senior Hospital in the Home, Ambulatory and Virtual Care Pharmacist, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
You’ll further have:
- Registered as a pharmacist with AHPRA
- Extensive hospital pharmacy experience
- Demonstrated advanced clinical pharmacy knowledge
- Knowledge of Hospital in the Home pharmacy practice
- Effective and courteous communication skills and the ability to work in a team
- Reliable and show due care and attention to accuracy and detail.
