About the Health Independence Program
The Health Independence Program (HIP) is made up of a number of programs including Fast Track Care Management, Older Persons Complex Care (OPCC) team, community rehabilitation, specialist services and clinics, and care management.
The Fast Track Care Management Team is a highly capable and energetic multidisciplinary team of nursing and allied health professionals with support from geriatricians that provides short term care management and clinical support for vulnerable clients in the community. We are always client-centred and work as a team.
About the role
This position provides acute clinical assessment and management of older clients in the community to facilitate early discharge and to prevent admission. Working with nurse practitioners and geriatricians you will use your advanced assessment and clinical skills to manage older people in the community.
This is a great opportunity to take your already great skills further, and will perhaps help you consider working towards a nurse practitioner qualification. No weekends or public holidays required.
A regular day will see you:
- Provide a rapid and flexible outreach response for the HIP eligible client and work closely with that client in the community (mostly short term) under the direction of the Older Persons Complex Care (OPCC) Nurse Practitioner
- Assist with low intensity short term care coordination for the older person with chronic and complex needs in the community
- Review and evaluate each client’s care plan and clinical progress until discharge
About you
As the Registered Nurse – Older Persons Complex Care, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
To be successful in this role, you'll bring with you:
- Clinical experience working with clients with chronic, aged, and complex health needs including behavioural and psychological symptoms of dementia, acute delirium, aged-related conditions, palliative and end of life care
- At least five years post-graduate nursing experience in clinical areas that are relevant to this position.
- An advanced level of clinical knowledge and expertise relevant to the area- including acute aged care, aged care.
