About the Physiotherapy Department
The Physiotherapy Department is committed to delivering the highest levels of care for our patients. We lead and excel in research, teaching and person-centred, evidence-based practice, providing services across the continuum from Emergency through bed-based services to ongoing ambulatory care.
About the role
As a Grade 2 Physiotherapist in Sub-acute Care (Geriatric Evaluation and Management – GEM), you will manage an individual caseload of frail and complex older adults, delivering safe, effective and person-centred physiotherapy. You will work closely with the multidisciplinary team to support complex discharge planning, communicate functional trajectory and recovery potential, and contribute to falls prevention and high-value care across the service.
A regular day will see you:
- Manage a complex GEM caseload, including older adults with geriatric syndromes, frailty, orthopaedic presentations (including acute fractures) and general medical issues
- Undertake assessment, treatment planning and delivery of evidence-based rehabilitation with a strong focus on safety, function and participation
- Support complex discharge planning by accurately predicting and communicating functional trajectory and recovery journey to the MDT, patients and families
- Have crucial, respectful conversations when progress is limited and a compensatory approach is required
- Provide detailed clinical handover and maintain timely documentation in the electronic medical record
- Contribute to falls prevention and management strategies for patients and the wider team
- Support the development of Grade 1 physiotherapists, AHAs and students through supervision, orientation and informal teaching as required
About you
You are an experienced physiotherapist who enjoys working with frail, complex older adults and brings strong communication skills, initiative and flexibility. You practise collaboratively, contribute positively to team culture, and are motivated to continue developing expertise in geriatric rehabilitation, frailty management and functional prognostication.
To be successful in this role you’ll bring with you:
- Current AHPRA registration and tertiary qualification in Physiotherapy
- Experience managing frail/complex older adults, including complex discharge planning
- Excellent communication and proven ability to work effectively within a multidisciplinary team
- Sound understanding of clinical systems and digital documentation
- Commitment to ongoing education and evidence-based practice
