About the Health Independence Program (HIP)
Sitting within the Continuing Care Division, HIP is a group of programs supporting patients/clients to be as independent as possible at home and avoid hospital admission. We pride ourselves on our strong patient-centred focus and in providing best practice and innovative care to our patients/clients.
About the role
The Grade 2 Physiotherapist is a key position in the Fast track Care Management team, providing direct support for our clients in the community and working with team members to ensure their clients develop/maintain optimal function at home.
A regular day will see you:
- Working with clients in their homes and from the office
- Enjoying the camaraderie of multidisciplinary colleagues to provide comprehensive patient/client care planning and support
- Learning from your multidisciplinary colleagues, all of whom are experienced, innovative and collaborative.

- Have extensive clinical experience in assessment, treatment and goal setting of highly complex older people
- Have the ability to motivate, mentor and inspire
- Have a collaborative spirit, and ability to build strong networks with peers
- Have excellent communication and decision-making skills
