About the Department

The Medication Management service is responsible for providing a comprehensive community-based medication review, aiming to minimise medication misadventure in the community. The position is primarily based at the Heidelberg Repatriation Campus and includes travel between cleints' homes and Twin Parks residential aged care facility in Reservoir.   

 

About the role

The Grade 3 Pharmacist will work in an integrated role as a member of the Medication Management team and the Transition Care team. The Grade 3 Pharmacist will provide a high standard of clinical practice in accordance with clinical guidelines and ensure care and education is planned and delivered in partnership with the patient/carer to acheive the best possible outcomes. 

 

A regular day will see you:

  • Assess clients referred to the Medication Management team and Transition Care team and implement and evaluate treatment plans as required.        
  • Devise intervention priorities and goals in consultation with the client, family or caregivers.  
  • Provide education and consultation to clients and their carers, GPs, other health professionals and community agencies, where relevant, to ensure continuity of care. 
  • Work as an effective member of the HIP team, assisting clients and their carers to navigate the health service system and contribute to the provision of high quality, accessible, flexible and client-centred service provision.   
  • Provide evidence-based management of clients with chronic diseases. 
  • Participate in the ongoing development and research in the medication management service. 

 

About you 

As the Grade 3 Pharmacist, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your clinical and leadership skillset.

 

You’ll also have:

  • Minimum of 5 years’ experience as a registered pharmacist.  
  • Experience as a clinical ward pharmacist.  
  • Demonstrate well developed assessment and medication review skills.  
  • Understanding of community pharmacy practice.  
  • Client centred approach to service delivery.
