About the Health Independence Program (HIP)

HIP is part of the Continuing Care Division at Austin Health and provides a range of multi-disciplinary supports and services for clients to transition safety home from hospital to the community. We support people with health problems and other complex conditions to optimise their function and self-management skills so that they can live independently and well in their homes and community.

About the role

As the Grade 2 HIP Fast Track Care Management Occupational Therapist, you will enjoy the support of the multi-disciplinary team as well as other Occupational Therapists working in HIP.

As one of the two Occupational Therapists in the team, you will work closely with the Complex Care Managers and Physiotherapists to ensure that our clients are well supported at home. The Occupational Therapist is an integral member of the team, assessing clients and arranging ongoing equipment and service needs.

The Occupational Therapist will have ongoing support from the Complex Care Managers, a Geriatric Fellow and the Central Intake and Fast Track Care Management Team Leader.

A regular day will see you:

• Consulting with the HIP Care Managers regarding client needs adn provide clinical guidance and support
• Provide expert advice to the Short Term Supports team to safe support the clients discharge and promote successful transitions home after hospital presentation
• Undertake home visits with clients and their support person to assess care needs, provide advice, and support clients to remain safe and independent at home.
• Enjoy the camaraderie of an experienced, energetic and innovative group of multi-disciplinary healthcare professionals.

As the Occupational Therapist you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.

To be successful in this role, you'll bring with you:

• Great communication skills across levels of the healthcare team, and clients and their families/ carers.
• Knowledge of the Australian Aged Care system and hospital discharge processes.
• Familiarity with contemporary Occupational Therapist management and evidence based practice.
