• Permanent, part time position available for a suitably experienced Registered Nurse in our Heart Failure Rehabilitation Program
• 12 hours per week (Mondays and Thursdays)

About the Health Independence Program department, Continuing Care Division
The Continuing Care division includes aged care, rehabilitation, community services (Health Independence Program (HIP), Residential In-reach Services, and Better@Home) and specialist clinics, with services provided across the Austin, Heidelberg, Royal Talbot campuses and into the community. 

The Health Independence Program (HIP) provides multi-disciplinary services that support people to optimise their function and self-management skills so that they can live independently and well in their homes and community. HIP Services aim to provide integrated and coordinated care that responds to the client’s needs and goals. Services are delivered at the centre, home, telehealth or community setting, and are time-limited.

The Heart Failure Rehabilitation Program is a multidisciplinary program, based at the Royal Talbot campus, and is coordinated by the Heart Failure Rehabilitation Co-ordinator.  

About the role

The Liaison Nurse - Heart Failure Rehabilitation supports the Heart Failure Rehabilitation program by providing nursing expertise to those attending the program. This role supports with assessments of patients referred to the Heart Failure Rehabilitation program, post program reviews (at 8 weeks) as required, and at 6 and 12 months, as required. The nurse will actively participate with the multidisciplinary team, and in partnershp with the patient, to plan and adapt care according to their heart failure rehabilitation goals.

A regular day will see you:

• Support the team with initial assessments of patients referred tothe Heart Failure Rehabilitation Program, and post program reviews as required.
• Supervise, monitor and educate patients under gymnasium-based exercise and home exercise.
• Suport patients undertaking home exercixe programs (via phone or Health Direct video calls) using motivational techniques to elicit behaviour change and improve self-management of the patient;s heart failure.
• Maintain Heart Failure Rehabilitation Program documentation for each client.  
• Liaise with other health professionals within the Rehabilitation team to ensure that client care is delivered through an integrated, multi-disciplinary approach.
