About Hospital in the Home & Virtual Care
The Hospital in the Home & Virtual Care program facilitates the shift of traditional acute care to the patient’s own home without compromising patient care. This care may take place via in-home nursing and support services, via video telehealth or via admission to the Hospital in the Home – Virtual Care, where higher acuity care can be provided in part through the utilisation of wearable technology. Hospital in the home is a supportive team, working collaboratively to ensure that people can have their care in the right place.
About the role
The HITH Visiting Nurse (Grade 3- YU15) provides high quality, evidenced based care to clients in a community setting. They can assess, treat, and escalate in the case of deterioration appropriately while managing the care of multiple patients independently. This person will be able to work collaboratively with the team in the provision of complex care in the home setting.
They are able to assist with the facilitation of telemedicine reviews & virtual care and are able to communicate clinical information from the home to the care team in a highly effective way.
A regular day will see you:
- Driving to visit admitted patients in their own home
- Conducting a wide range of care - from administration of IV medications via a PICC, assessing and treating people with heart failure, complex wound management, and assisting with virtual reviews.
- Assessing for deterioration at home, escalating to the case management and HITH medical teams to adjust treatment plans or handover for readmission
- Supporting discharge to the community beyond the HITH admission
About you
As the HITH Visiting Nurse, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
To be successful in this role, you'll bring with you:
- Excellent clinical assessment skills and experience, able to make decisions and escalate as needed
- A flexible approach - this role is growing and changing, with a broad range of skills required and new care types being added all the time.
- An enjoyment for working independently, with the backing of a supportive team.
- A comfort with using technology - you will be documenting on a number of clinical systems, and supporting patients under the Virtual model as needed.
