About Hospital in the Home and Virtual Care

Hospital in the Home and Virtual Care (HITH-VC) is a multi-disciplinary team providing seven day/week home-based services and aligns with a key direction of providing more care outside the hospital. Within HITH-VC a new model of care has been developed to support higher acuity patients utilising virtual care.Virtual Care operates to care for patients with cardiac conditions including heart failure, atrial fibrillation and post cardiac surgery care, and haematology conditions, including leukaemia and autologous stem cell transplants, caring for them in their home environment, in a multidisciplinary team setting.

 

About the role 

As a Hospital in the Home and Virtual Care (HITH-VC) Physician, you will be responsible for the following clinical responsibilities:

  • Friday HITH-VC consultant of the day including 0900 huddle via Microsoft Teams, supervision of junior medical staff and providing comprehensive management and multidisciplinary care to patients under HITH-VC.
  • Weekend and public holiday (remote) work in approximately a 1:4 roster including attendance at a daily 0905 huddle via Microsoft Teams and available for calls / messages from junior medical staff and case managers (nursing).
  • Educational support and supervision of junior medical staff rotated to HITH-VC.
  • Assist with the development and delivery of new care pathways, research and audit activities within the unit.

 

About you 

To be successful in this role you’ll bring with you:

  • Be a registered Medical Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
  • Hold a Fellowship of the Fellowship of the Royal Australasian College of Physicians (FRACP) in an adult medicine specialty
  • Hold certification of FRACP continuing professional development (CPD)
  • Hold AHPRA specialist registration
  • Demonstrate a commitment to high quality patient care
