About Hospital in the Home & Virtual Care  

Hospital in the Home & Virtual Care supports patients to have care in the safety and comfort of their own home. This care may take place through in-home nursing care and support services, or through admission to HITH Virtual Care, where higher acuity care can be provided through video telehealth reviews and the use of wearable technology.   

 

The HITH & Virtual Care Team is proud to be supporting the delivery of admitted care as part of the Victorian Virtual Hospital Pilot, supporting specialist care virtually in the comfort of home via VideoTelehealth reviews and utilising wearable technology.   

 

About the role 

The HITH Visiting Nurse provides high quality, evidenced based care to clients in a community setting. They can assess, treat, and escalate in the case of deterioration appropriately while managing the care of multiple patients independently. You will be able to work collaboratively with the team in the provision of complex care in the home setting.       

 

You will assist with the facilitation of telemedicine reviews & virtual care and be able to communicate clinical information from the home to the care team in a highly effective way.       

 

A regular day will see you:   

  • Driving to visit multiple patients, providing care from Heart failure management, medication management via Central Venous access devices, complex wound management, managing symptoms of a person post Bone marrow transplant supported by Virtual and in person care - and everything in between!
  • Managing patient deterioration or clinical changes effectively and independently from the home and escalating appropriately to the clinical teams in the hospital.
  • Supporting care by virtual means - providing in-person support for virtual care provision, facilitating video telehealth reviews, and providing the elements of care that need a clinician while being supported by the team at Austin
  • No two days are the same - the patients, acuity and care delivery varies from patient to patient and day to day - so this role will see you able to develop skills in many clinical areas and learn and grow as a clinician. 

 

About you 

You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset. 

 

To be successful in this role you’ll bring with you:

  • Registered Nurse registration with strong clinical background
  • The ability to confidently manage a highly varied patient population, assess changes in clinical condition and manage deterioration independently in the community.
