About the Hospital in the Home & Virtual Care department

Hospital in the Home & Virtual Care supports patients to have care in the safety and comfort of their own home. This care may take place through in-home nursing care and support services, or through admission to HITH Virtual Care, where higher acuity care can be provided through video telehealth reviews and the use of wearable technology. 

 

About the role 

We are looking for a passionate, experienced nurse who can coordinate safe, seamless care from hospital to home - triaging referrals, supporting patients and leading virtual care delivery.  

  

Work with nurses, doctors and allied health to keep patients safe, comfortable and cared for where they belong - at home.   

 

A regular day will see you: 

  • Coordinating patient assessments to provide home nursing services, facilitating video telehealth reviews, and completing complex discharge planning. 
  • Supporting escalation of the deteriorating patient - both through supporting visiting nurses remotely, and through the provision of on-call support and after-hours escalation 
  • Formulating individual plans for patients to support their complex needs, aiming to provide care virtually where possible and supporting the patient and the treating team with a high level of confidence and expertise.  

 

About you 

You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work and have a keen interest in developing your skillset. 

 

To be successful in this role you’ll bring with you:

  • A strong level of clinical experience and confidence, allowing you to make excellent decisions on clinical escalation and care provision 
  • A collaborative mindset, working with the treating teams, care partners, the patient and their carer to provide high quality, evidence-based care at home.  
  • A flexible, person centred approach to care, implementing care plans for patient to allow them to get home as soon as possible.  
  • A current, unrestricted Driver's license
