About the Aged Care Assessment Services
The Aged Care Assessment Services sit within Aged Care Community Services as part of the Continuing Care Division at Austin Health. The service currently undertakes My Aged Care comprehensive assessments for Commonwealth Government subsidised services (Residential Care, Residential Respite Care, Home Care Packages, Transition Care and Short-Term Restorative Care) in both hospitals and in the client’s home. From early December our program will also provide assessments for home support to clients in the community.
It is a service that helps older people and their families make decisions about the services that they need to remain at home safely.
About the Role
The Home Support Assessor is a non-clinical role that works alongside clinical assessors as a component of the My Aged Care (MAC) assessment workforce. The Home Support Assessor will provide phone, telehealth and in home (face to face) assessments of clients referred via the MAC Gateway using the Integrated Assessment Tool (IAT).
A regular day will see you:
- Providing kind and friendly client contact and support
- Undertaking screening and assessments of clients using the IAT
- Working with clients to establish goals, developing a support plan and where required supporting them to link in with services
About you
You will be someone who has excellent communication and interpersonal skills; strong report writing skills and attention to detail; who is able to work autonomously, as well as in a team environment; who is willing to learn and develop their skillset, and who wants to work in a program that makes a difference to clients in our community
