About the Health Independence Program department, Continuing Care Division
The Continuing Care division includes aged care, rehabilitation, community services (Health Independence Program (HIP), Residential In-reach Services, and Better@Home) and specialist clinics, with services provided across the Austin, Heidelberg, Royal Talbot campuses and into the community.
The Health Independence Program (HIP) provides multi-disciplinary services that support people to optimise their function and self-management skills so that they can live independently and well in their homes and community. HIP Services aim to provide integrated and coordinated care that responds to the client’s needs and goals. Services are delivered at the centre, home, telehealth or community setting.
The Central Intake and Fast Track Care Management team provide a single point of entry for all HIP service referrals, and for the HIP eligible client, this team provide, on referral:
- Support (up to 28 days) after hospital discharge, including arrangement and coordination of short-term domiciliary services
- Short-term case management (up to 12 weeks) to support clients with more complex or longer-term needs, and
- Clinical support, education and timely specialist care of the Older Persons Complex Care service.
About the role
Reporting to the (HIP) Chronic and Complex Care Program Manager, the key function of the role is to coordinate and oversee the day to day running of the HIP Central Intake and Fast Track Care Management service (which includes the Older Persons Complex Care Service) ensuring that key service performance deliverables are met including consumer outcomes, activity targets, human resource and quality. The HIP Central Intake and Fast Track Care Management Team Leader will also have a clinical role within the team.
A regular day will see you:
- Coordinate the HIP Central Intake and Fast Track Care Management team including recruitment and selection, daily staffing, job rotation, performance and leave management, rostering, and attendance.
- Supervise and monitor the performance of all functions performed by the HIP Central Intake and Fast Track Care Management staff.
- Act as a leader and role model for the team, setting and clearly communicating expectations.
- Manage, liaise and work collaboratively with other HIP team leaders and staff to provide optimum service delivery.
- Review of existing practices, policies, and procedures according to evidence-based practice to minimise adverse outcomes.
About you
As the HIP Team Leader - Central Intake and Fast Track Care Manager, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
To be successful in this role, you'll be a:
- Registered Nurse (currently registered with Nursing and Midwifery Board of Australia) or Allied Health Professional (current registration with their respective National Board, or eligible for membership of relevant professional association).
- 7 years relevant clinical experience (note: allied health professional requirement)
- Previous management experience of clinical staff in a health setting.
- Demonstrated leadership ability.
- Proven ability to problem solve in a variety of complex situations.
- Well-developed ability to identify opportunities for service improvement, implement, lead, and support change.
- Have a demonstrated ability to use clinical and operational information systems effectively and efficiently.
