About Residential InReach (RIR) Team
The Residential InReach (RIR) team is dedicated to delivering the highest standard of person-centered care to our patients. The team is composed of clinical nurse consultants, aged care advance trainees, and geriatricians who provide rapid response services to residents within residential aged care homes (RACH). Their goal is to optimise residents' health and prevent unnecessary hospital admissions.
About the role
As the clinical nurse consultant in the RIR team, you will be responsible for delivering evidence based expert advice and clinical support for residents within aged care homes to optimise patient outcomes and minimise presentations into the emergency departments.
A regular day will involve you:
- Supporting the triaging of referrals from staff at Residential Aged Care Homes (RACH) to ensure prompt assessments and provide short-term support to prevent unnecessary admissions to acute care when possible.
- Conducting comprehensive nursing assessments, including reviewing common conditions such as infections, delirium, medication reviews, and advanced care planning discussions, including end-of-life care.
- Offering education and support to hospital staff for discharge planning, ensuring a smooth transition back to RACHs.
- Providing education and guidance to RACH facility staff on managing specific resident issues within the care homes.
About you
As the RIR CNC, you will be someone with a high level of assessment skills, excellent communication skills, with a strong commitment to excellence in acute patient care.
You’ll further have:
- Nursing and Midwifery Board of Australia Registered Nurse registration
- Extensive acute aged care or Emergency nursing care experience
- Current Driver's license
- Meet the selection criteria within the position description
- The ability to work independently or within a high functioning team to meet varying patient demands.
