About the Mental Health Division
The Mental Health Division is committed to promoting the highest levels of care for our patients. In realising this goal, the department will lead and excel in research, teaching and the use of person-centred evidence-based practice. The department provides services on all three campuses of Austin Health, from the Emergency Department through to bed-based services and ongoing ambulatory care.
The Adult & Older Adult Mental Health Service provides comprehensive mental health care for individuals aged 26 years and over, as well as support for their families and carers. The service operates across the City of Banyule and Shire of Nillumbik and forms part of the broader Mental Health Division.
About the role
The Continuing Care Service (CCS) is a multidisciplinary team of highly passionate, experienced, supportive and collaborative mental health professionals. This Case Manager position will focus on specialist care for CCS consumers with co-morbid eating disorder diagnoses. Dependent on demand, this position will have the flexibility to work with a wide range of mental health presentations & draw upon specialist eating disorder knowledge where indicated to provide:
- Improved access to community-based eating disorders care via increased service capacity
- More holistic mental health care for people with eating disorders
- Provide a clear assessment and treatment pathway for consumers and families/carers
A regular day will see you:
- Share your expert opinion and experiences to our team to help make a difference
- Assess and treat our clients, liaising as appropriate with family members and caregivers to promote patient care
- Collaborate with multiple teams across the division and wider community
- Support the care and recovery of peoples living with eating disorder diagnoses and other diagnosed mental illness
- Advocate for your clients right to access safe and timely care, at the right place at the right time
About you
As a Case Manager, you will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. To be successful in this role, you'll bring with you flexible thinking, creative ideas and a passion for working within a strong multidisciplinary team to make a real impact on clinical outcomes.
To be successful in this role you’ll bring with you:
- Experience and skills in delivering evidenced-based assessment and treatment of eating disorders
- Minimum 5 years’ experience in a relevant mental health setting - including eating disorder inpatient experience &/or outpatient community experience
- Demonstrated knowledge of recovery and collaborative clinical practice.
- A working knowledge of the Mental Health & Wellbeing Act (2022), the Carer Recognition Act (2012), and Chief Psychiatrists Guidelines
