  • Case Manager – Adult & Older Adult Community Mental Health
  • Permanent, full-time role based in Heidelberg with a monthly ADO  
  • Open to Mental Health Professionals with following qualification: Registered Nurse (Div1), Clinical Psychologist, Social Work, Occupational Therapist 
  • Member of the North East Area Mental Health Service (NEAMHS) Continuing Care Service (CCS)

About The Mental Health Division

The Mental Health Division (MHD) provides mental health assessment and interventions for people experiencing mental illness and their families and carers. Working from a recovery-oriented model, the Mental Health Division provides an array of hospital-based, community and specialist services for infants, children, young people, adults and older adults. 


About the role

CCS is a multidisciplinary team of highly passionate, experienced, supportive, collaborative and enthusiastic mental health professionals. Our values underpin everything we do. You will form solid working-relationships within the pre-existing community teams and foster strong therapeutic relationships with your clients & their families/carers. You’ll join a friendly and supportive team who genuinely enjoy their work and the contribution they make.

 

A regular day will see you:

  • Assess and treat our clients, liaising as appropriate with family members and caregivers to promote excellent clinical care
  • Collaborate with multiple teams across the division and wider community
  • Support the care and recovery of peoples living with complex mental health diagnoses and presentations
  • Advocate for your clients right to access safe and timely care, at the right place at the right time

 

About you

As a Case Manager, You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.

 

To be successful in this role, you'll bring with you:

  • A commitment to working in a strong team & a desire to support & grow with your colleagues 
  • Demonstrated knowledge of recovery and collaborative clinical practice.
  • A working knowledge of the Mental Health & Wellbeing Act (2022), the Carer Recognition Act (2012), and Chief Psychiatrists Guideline
