Austin Health is the major provider of tertiary health services, health professional education and research across three campuses in the north-east of Melbourne and statewide. We have a highly engaged, diverse and collaborative workforce that is unified by our goal to help people live healthy and fulfilled lives.

About the Allied Health Division

The Allied Health Division employs over 1,000 health professionals across 27 disciplines, broadly known as the therapies and sciences, who work as key members of the multidisciplinary team aiming to provide the best outcomes for patients by preventing, diagnosing and treating a range of healthcare conditions.

Allied Health staff support patients at every stage of their healthcare journey across Austin Health with services provided across all sites (acute and sub-acute), in the ambulatory setting, at home and in the community.

About the role

We are seeking an experienced and innovative Allied Health leader to establish and lead our new Allied Health Specialist Assessment and Pathways Service.

This is an exciting opportunity to shape a specialised service focused on supporting patients with complex discharge planning and progression of care needs. Working across Austin Health's acute campus, you will lead a small team providing expert consultation, advanced assessment and developing creative solutions for patients experiencing psychosocial, functional, disability-related or community support barriers to discharge.

As the service lead, you will work closely with multidisciplinary teams, community providers and external agencies to facilitate timely, patient-centred transitions from hospital to home and community-based care. A strong focus of the role is supporting staff to support patients with prolonged hospital stays, complex disability needs, NDIS requirements, aged care transitions and hospital avoidance initiatives.

You will also play a pivotal role in designing and embedding the service, using data-informed evaluation, quality improvement and stakeholder engagement to ensure a sustainable and effective model of care.

A regular day will see you:

  • Lead and provide advanced clinical oversight for a specialised Allied Health consultation team focused on streamlining complex discharge planning and progression of care.
  • Provide consultation and support to Allied Health clinicians managing complex discharge planning and community transition needs (beyond the Allied Health Specialist Assessment and Pathways Service).
  • Conduct comprehensive patient assessments, including clinical file reviews and direct patient assessments, to identify barriers to discharge and opportunities for care progression.
  • Apply a broad Allied Health lens, using extensive knowledge of the roles of the multidisciplinary team to consultation scenarios, to consider psychosocial, functional and physical barriers to discharge.

About you

You are an experienced Allied Health clinician with:

  • A minimum of 10 years' clinical experience in Allied Health practice
  • Extensive experience working within a public hospital setting
  • Advanced expertise in complex discharge planning, care coordination and progression of care
  • Strong knowledge of NDIS and Aged Care pathways, community services and hospital avoidance strategies
  • Demonstrated success leading teams and building workforce capability
  • Excellent communication, negotiation and stakeholder engagement skills
  • Experience using service data, evaluation and quality improvement methodologies to drive outcomes
  • Tertiary qualifications in an Allied Health discipline and current AHPRA registration (or equivalent professional registration)
