About the Austin Health Aged Care Assessment Services
The Austin Aged Care Assessment program sits within Aged Care Community Services as part of the Continuing Care Division at Austin Health. The Service undertakes both comprehensive and home support assessments for eligible older Australians as part of the new Single Assessment System for Aged Care. It is a service that helps older people, and their families make decisions about the services that they need to remain at home safely.
About the role
As an Aged Care Assessor you will work as part of the Aged Care Assessment team, undertaking detailed individualised assessments and care planning for eligible older clients.
A regular day will involve:
- Conducting comprehensive assessments utilising the Integrated Assessment Tool and providing a choice of appropriate services.
- Providing information and referring clients to appropriate and available services, including facilitating access to broader community services including CHSP, Mental Health and NDIS.
- Working closely with the Team Leader to support program achievement of Commonwealth and State reportable KPI's
About you
You will be someone who has excellent communication skills, with a strong commitment to excellence in patient care. You’ll bring energy and drive to your work, and have a keen interest in developing your skillset.
You'll further have:
- Current registration as a Registered Nurse (currently registered with Nursing and Midwifery Board of Australia) or Allied Health Professional (with current registration with their respective National Board) or eligible for membership of relevant professional association
- Strong assessment and clinical skills and demonstrated experience in an aged care or community setting
- Excellent interpersonal and communication skills that support patient care, team engagement and collaboration
- A willingness to think outside the square and think creatively to achieve the best possible client and program outcomes
- Demonstrated flexibility to change and adapt with program and organisational requirements
